中新网南昌5月15日电 (记者 吴鹏泉)“以前乡镇卫生院没有康复治疗，儿子和儿媳隔几天就要送我去中医院治疗，耗时耗力耗钱，现在镇里卫生院也能进行康复治疗，我自己就可以骑车过来了。”
前不久，患有腰椎间盘突出症的江西省丰城市拖船镇居民徐九香再次来到镇卫生院接受治疗，卫生院康复科医师为其提供了全方位医疗服务。经过一段时间治疗，这位年过古稀的老人病情得到有效控制，身体状况明显改善。
“小病不出乡、大病不出县”这是很多基层民众看病就医的愿望。近日，记者深入江西多地采访，探寻这片革命老区是如何通过强化基层医疗卫生服务，方便民众“家门口”就医。
优质医疗资源持续下沉
初夏时节，在江西省九江市柴桑区江洲镇官场村颐养之家，老人们或在广场上散步闲谈，或在棋牌室打牌娱乐。“有事到村部，有病到村卫生室，走路只要一分钟。”住在颐养之家的王长娇笑着说。
江洲镇地处长江中心孤岛，进出靠轮渡，就医不便，医疗水平有限。为将省级优质医疗资源下沉，江西省人民医院自2021年起向官场村开展“远程医疗、送医入户、定期义诊”三项健康帮扶措施。
“医院面向村民优先满足床位，减免检查、医药费用。针对行动不便或身体虚弱的村民，我们专家团队入户给予体检问诊，拿出‘定制化’诊疗方案。”江西省人民医院副院长邹清水说。
当前，中国民众就医服务需求呈现多样化、个性化，对基层医疗机构也提出了更高要求。
2023年11月，江西省政府办公厅印发《关于进一步深化改革促进乡村医疗卫生体系健康发展的实施意见》(以下简称《实施意见》)，提出依托紧密型县域医共体建设，发挥医共体牵头医院的帮扶和带动作用，推进优质医疗资源下沉，全面提升乡村医疗机构服务能力。
江西贵溪市是全国紧密型县域医共体试点县和全省综合医改试点县。贵溪市卫健委主任叶剑锋介绍说，为整合区域医疗卫生资源，当地组建了贵溪市总医院，统筹管理辖区内5个院区、21个基层医疗机构和171个公有产权村卫生室。
“市总院定期安排专家、业务骨干前往基层进行指导，我们乡镇可以进行一、二级手术。”贵溪市总医院塘湾分院院长王鹏说，得益于县域医共体改革，乡镇卫生院的医疗服务条件有了巨大变化。
基层卫生人才增量提质
做好基层医疗卫生服务，具体实施者是基层医务人员。
记者了解到，江西建立基层引才用才倾斜政策，明确以县为单位，乡镇卫生院编制岗位数“打捆”设置、统筹使用，按照“有编即补”的原则优先保障基层医疗卫生机构用人需求。
江西还创新村医管理机制，乡村医生实施“乡聘村用”；建立乡村医生多渠道补偿机制，该省年满60周岁且从业满20周年的乡村医生，每月发放300元养老生活补贴，引导老年村医有序退出；实施大学生乡村医生专项招聘，为村卫生室补充合格人才。
叶剑锋称，为解决人才从哪来，如何留下来的问题，贵溪市重点开展了“县管乡用”“乡聘村用”和人事薪酬三大改革，推进基层卫生人才增量提质。该市基层医疗机构可提取医疗服务性收入的60%用于人员奖励，年终若有收支结余，结余部分的50%仍可用于绩效奖的二次发放。
统计数据显示，截至2022年底，江西全省乡镇卫生院1597所，卫生人员54885人，村卫生室26136个，人员总数45446人；全省社区卫生服务中心(站)597个，社区卫生服务中心卫生人员6281人，社区卫生服务站卫生人员2997人。
曾宝是江西省赣州市安远县一名扎根基层数十年的乡村医生。为提高医学技能，他挤出时间看书，并积极参加各种培训，最终取得了执业医师、执业药师双职称，“了解越多，才能救助更多人”。
农村地区医保水平提高
“以前来医保服务大厅办业务，需要跑好几个窗口，现在可不一样了，只要在大厅任何一个窗口提交材料，很快就能办结。”近日，到江西省上饶市余干县政务服务大厅医保窗口办理业务的万丽华感到很欣喜。
受益于“综合柜员制”，万丽华的业务才办得如此顺利。在“综合柜员制”实施过程中，余干县医疗保障局围绕民众关心的问题，梳理业务流程，逐项解决了办理流程、办理时限、业务交叉、前后台衔接等问题，提高了事项办理效率。
余干县医疗保障局实施“综合柜员制”，是江西提高农村地区医疗保障水平的一个生动缩影。
根据《实施意见》，江西将实现村卫生室医保结算，将符合条件的村卫生室纳入医保定点管理；合理提高医保基金对乡村医疗卫生机构的总额控制指标，年度新增医保基金重点向乡村医疗卫生机构倾斜，逐步提高县域内医保基金用于乡村医疗卫生机构的比例。
江西还着力加强农村地区医保经办管理服务和监督管理能力建设，探索将村级医保服务纳入农村网格化服务管理。
“健康是幸福生活最重要的指标，也是实现共同富裕的基础保障。”江西省卫健委主任龚建平此前表示，江西将落实好以基层为重点的工作方针，全面推开紧密型县域医共体建设，落实好推动优质医疗资源下沉和均衡布局的政策措施，解决好区域协调、城乡协调、中西医协调、医防协调发展等方面问题。(完)
